Medical Leave of Absence Extension

ALL students who have completed two consecutive semesters of medical leave are required to request an extension if they are not ready to return to their academic program.
- Requests for an extension should be made by the end of your second semester of leave.
- You may choose to include documents supporting your extension request.
- Failure to submit an extension request may impact your future ability to return to your program.
Once your request has been made, you will be contacted by the Office of the Dean of Students for a meeting to discuss your request and address any concerns you may have.
Forms to Request a MLOA Extension
MLOA Extension Request Form – Completion of this form is required to request an extension if you are not ready to return to your academic program.
MLOA Additional Documentation Submission Form – The purpose of this form is to provide an additional method for submitting documentation should you wish to submit anything in support of your request to extend a medical leave of absence.
Release of Information – This is a fillable PDF that will allow the Dean of Students and/or Student Outreach and Support to speak with your healthcare provider in the event more information is needed to review your MLOA Extension Request.
