Medical Leave of Absence Extension
Graduate students who have completed two consecutive semesters of medical leave are required to request an extension if they are not ready to return to their academic program. Do not complete this process if you are an undergraduate student.
- Requests for an extension should be made by the end of your second semester of leave.
- You request must include a letter of support from your treating healthcare provider.
- Prior to January 2025 it was required to use the Student Health Center or CAPS to submit this letter, however you may use your own community provider.
- Failure to submit an extension request may impact your future ability to return to your program.
Once your request has been made, you will be contacted by the Office of the Dean of Students for a meeting to discuss your request and address any concerns you may have.
